Status Request Form

This form is a Status requests.
Filling this form out and sending it, does not guarantee that you will be granted Status.
Approval will be determined at the Officer's meeting. 
Please read ARTICLE XXII PROCEDURE FOR CHANGE IN MEMBERSHIP STATUS to determine if you are eligible to change your status.

    For certain status changes documentation must be submitted. Please make sure you are aware of any documentation you will need in order for the status request to be approved.
